List of Documents to bring

Immigration applicant will need to bring the following to their examination:
‍

1. Government-issued photo identification (i.e. driver’s license or passport) for everyone, regardless of age.
2. Immunization records.
3. Form I-693 (either from your attorney or from the government’s website), the identifying information in Part 1 should be completed, but DO NOT SIGN or date the form until told to do so by the civil surgeon.)

You must be able to return to the office two-three days following your examination to have the TB skin test read. We will schedule a followup appointment for you.
